FormReturn is a feature-packed, integrated OMR software solution. Using the intuitive, built-in designer, you can create forms with OMR 'bubbles', bar codes, variable text (such as respondents names), and of course static elements such as question text, lines, boxes, and images/logos. Once published all forms carry a unique Form ID bar code. This bar code is later used to automatically recognize and read the forms.
Filled-in forms are scanned using a generic image scanner. Your organization may already have a document scanner. We'd be happy to help you assertain compatibility.
FormReturn can process individual images or monitor a specific folder and automatically read new images. Once processed, data is easily exported in the ubiqutous text format (comma or tab delimited) for further analysis with a spreadsheet or other program.
