1. Design and Print

We design half-page, full-page and multi-page forms with your logo and to your specification. Forms can include workshop/session identifiers and space for hand-written notes. Here are a few samples:

• NARHA, half-sheet; forms contain session title and are pre-slugged with machine-readable session ID (total of 40 different sessions).
• National Veteral Small Business Conference & Expo, half-sheet; forms contain session title and are pre-slugged with machine-readable session ID (total of 13 different sessions).
• VMware, first page of a 3-page survey;
• North Carolina Association of CountyDirectors of Social Services, half-sheet, overall-conference evaluation.

2. Data Capture (Scan) and Report

Once you return your filled-in forms we will scan them and send you an Excel file containing your captured data and one or more reports. We are committed to sending you the data file within three days of receipt of your filled-in forms. We can also enter hand-written comments made by attendees.

Reports are easily customized to meet your requirements. And because all your data is also included in the Excel file, you can analyze and graph to your hearts content. Here are a few sample reports:
